A relicensure survey was completed on 1/27/23. Deficiencies were cited. Based on record review and interview, the residence failed to have a readily available roster of current residents with their room assignments, emergency contact information and a residence diagram showing room locations, affecting seven current residents.Findings include:On 1/27/23 at approximately 9:20 a.m., the administrator designee (AD) stated she typically printed the resident' s facesheet when there was an emergency. She added there was no list of resident' s names, room numbers, or emergency contacts that she was aware of. On 1/27/23 at 10:33 a.m., Staff #2 printed and provided a resident roster. The resident roster included resident names, room numbers, admission date, a column titled Therapy, care plan date, a column titled LAL equipment, influenza vaccination status, coronavirus vac.. Based on record review and interview, the residence failed to have at least one staff member onsite at all times who had current certification in cardiopulmonary resuscitation (CPR) and obstructed airway techniques from a nationally recognized organization, affecting seven current residents.
