Based on document review and interviews, the facility failed to maintain, review, and update the Emergency Preparedness (EP) Plan annually. Failure to ensure the EP plan was reviewed annually poses a potential risk that all required revisions to the plan will not be recognized by residents and staff.
Based on observations and staff interviews, the facility failed to maintain resident room doors in the facility. Failure to maintain resident room doors could cause the door to fail during a fire which could harm the resident and/or staff.Â
Based on observation, it was determined that the facility failed to have the light switch 5 ft above the finished floor in one of the oxygen storage rooms. Failing to mount or protect the wall outlet receptacles from physical damage could cause harm to the patients and staff if an oxygen bottle accidentally hits the wall receptacle outlets.Â
