What this means for your family

While many families report a positive, clean environment with friendly staff, there is a recurring pattern of slow response times to call bells and inconsistent communication regarding medical needs. We strongly advise that you visit the facility at different times of the day—especially on weekends—and ask management specifically how they handle call bell response times and incident reporting for residents with dementia.

Laureldale Skilled Nursing and Rehabilitation Center receives highly polarized feedback, with many families praising the friendly, attentive staff and clean environment, while others report serious concerns regarding neglect, slow response times, and poor communication. While some visitors highlight a compassionate and professional team, multiple reviewers have documented issues with call bell response times, hygiene, and the quality of medical oversight. Prospective families should conduct an in-person tour and maintain frequent, unannounced visits to ensure their loved one's needs are consistently met.

Key Review Excerpts

Most of the nurses and aides are friendly but it takes no less than 45 minutes to answer a call bell. One night I rang the bell because I needed help getting out of bed and waited two hours for someone to help.

Rehab patient · 2023☆☆☆☆

My Grandfather had to be placed in a nursing home as taking care of him at home became too much for my Grandmother. Unfortunately, due to how short-staffed this place is, my Grandmother still has to come three times a day regardless of the weather or her own health because if she doesn't no one will bother to make sure my Grandfather eats.

Memory care family member · 2018☆☆☆☆

I was greeted immediately by the Administrator of Admission, who was helpful, friendly and very professional. The facility was clean, organized and well staffed.

This facility has a concerning pattern of recurring issues, with families filing multiple complaints that triggered 12 of the 24 total deficiencies. The most frequent problems involve safety hazards and accident prevention, resident protection from abuse and neglect, and proper treatment and care delivery. While all deficiencies show correction dates, the repeated nature of safety and abuse protection violations across multiple surveys and complaint investigations suggests ongoing challenges in maintaining consistent care standards.
