The facility underwent an unannounced Case Management visit to address an elopement incident report. The Licensing Program Analyst reviewed the LIC 421IM with the administrator, noting that the document was not signed at the time of the previous visit. No formal citations were issued during this visit.

The inspection was a case management visit following an elopement incident report. The primary finding is a failure to provide adequate supervision, which directly resulted in a resident leaving the facility unassisted. Two Type A deficiencies were cited related to the lack of required care and supervision for residents.

The case management visit identified one deficiency related to food service. Specifically, the observed quantity of food served was insufficient to meet the residents' daily nutritional needs. This deficiency is classified as Type B, requiring corrective action to ensure adequate food availability.
