Key Review Excerpts

The care staff is good, but the administrators seemed disorganized the few times I spoke to them. She was never bathed, she fell multiple amounts, they stack three patients into a tiny room, she was attacked twice by the same patient with no resolution.

Short-term resident's family · 2024☆☆☆☆

My niece encountered a patient of this facility on the street at 5:20pm wondering around the Fox Theatre. She had a bracelet on her arm that was not hers because when asked what her name was she said it was not the person on the bracelet she was wearing.

8 complaint-triggered

Families have filed complaints triggering investigations, including recent concerns about resident protection from abuse. This facility shows persistent issues with fire safety systems, medication management, and staff training that recur across multiple surveys. While the provider reports correcting each deficiency, the pattern of repeated violations in critical areas like electrical systems, fire alarm maintenance, and nursing competencies suggests ongoing operational challenges that families should carefully consider before placement.
