The most recent inspection in October 2025 identified issues regarding the documentation of medication reviews for long-term PRN use and failure to document physician notification following medication refusals. Previous inspections in 2023 and 2016 showed no deficiencies.
Staff did not consistently document that a health care professional was contacted when Resident D refused Lisinopril; additionally, there was no documentation that the doctor was contacted regarding the resident's request to speak to them.
