The facility underwent an unannounced case management visit on February 13, 2025. The Licensing Program Analyst toured the facility and met with the Administrator. No deficiencies were cited during this inspection.

The inspection, conducted following a temporary suspension of fire clearance, revealed significant safety concerns. Two Type A deficiencies were cited, specifically regarding the admission of residents without proper fire clearance and the facility's failure to maintain a valid fire clearance. Immediate corrective action is required to ensure resident safety.

The unannounced 1-Year Required inspection was conducted on 04/11/2024. The facility was toured, and general safety items like fire extinguishers and smoke detectors were noted. No deficiencies were observed during the visit, though the facility was asked to submit several required documents by 04/18/2024.

The annual inspection identified three deficiencies related to facility compliance. These include failure to maintain adequate liability insurance, an outdated fire extinguisher service tag, and an expired first aid kit. All cited deficiencies are classified as Type B, indicating non-compliance that requires corrective action.

The inspection was conducted on 04/20/2022 and found the facility not ready to accept residents. One Type B deficiency was cited regarding maintenance and operation, specifically citing unorganized resident rooms and needing service/replacement of fire extinguishers. The facility was advised to submit proof of corrections by the due date of 05/06/2022.
