Ivy Terrace at Fullerton

This report details two separate complaint investigations conducted on the facility. The first investigation regarding medication administration and meal supervision was found to be 'Unfounded.' The second investigation concerning elopement and falsifying reports was deemed 'Unsubstantiated.' No actionable deficiencies were cited in the final report.

This unannounced Case Management – Health Checks inspection was conducted by LPA Sean Haddad. The LPA toured the facility and observed that the facility was clean and organized, and no immediate health or safety issues were found with the residents present. The facility was found to have adequate supplies of food and properly stored medications, sharps, and toxins.

The inspection was a Required – 1 Year visit conducted on 07/21/2025. The primary deficiency cited relates to water temperature safety, as hot water was found at levels posing an immediate risk to residents. All other areas reviewed, including linens, food supply, and safety equipment, appeared satisfactory during the tour.

This was an unannounced Case Management visit conducted to follow up on an Incident Report from 09/17/2024. The investigator found the facility clean, safe, and sanitary, and observed no apparent signs of distress or injuries on the resident in question. No deficiencies were cited per Title 22 Division 6 of the California Code of Regulations.

The inspection revealed multiple deficiencies across several critical areas. Most notably, there are immediate safety risks concerning fire clearance capacity for non-ambulatory residents and the employment of an uncleared staff member. Additionally, the facility failed to maintain proper administrative designation, required safety equipment (evacuation chair), and has outstanding licensing fees.
