Immaculate Heart of Mary Assisted Living Home

Based on observation, record review, and interview, the manager failed to ensure a personnel record was available for one of three employees sampled. The deficient practice posed a risk as required information could not be verified for E3. Findings include: 1. Upon arrival, the Compliance Officer was greeted and invited into the facility by E3. 2. While onsite, the Compliance Officer observed E3 providing unsupervised personal care services for residents. 3. The Compliance Officer requested E3's personnel record for review. E1 and E2 reported E3 was recently hired and did not have a personnel record for review. The Compliance Officer reviewed a fingerprint clearance card application submitted on August 16, 2024. There was no additional information for review. 4. In an interview, E1 acknowledged there was no personnel record available for E3.
