The facility failed to maintain an environment free of hazards due to improperly stored oxygen canisters. Specifically, unsecured canisters were observed on the floor in the Administrator's office, near Room 31, and in a resident's room. This practice violates the facility's own policy requiring cylinders to be stored upright and secured to prevent falling.

The facility failed to ensure that follow-up blood pressure results were reported to the physician according to prescribed parameters for a resident. Specifically, blood pressure readings that met the criteria for physician notification were documented in the resident's logs but were not communicated to the licensed practitioner.
