The facility failed to maintain an environment free of hazards due to improperly stored oxygen canisters. Specifically, unsecured canisters were observed on the floor in the Administrator's office, near Room 31, and in a resident's room. This practice violates the facility's own policy requiring cylinders to be stored upright and secured to prevent falling.

The facility failed to ensure follow-up blood pressure results were reported to the physician according to prescribed parameters for a resident. Review of medication records and blood pressure logs from October through December 2016 showed multiple instances where systolic and diastolic readings fell outside the ordered range without evidence of physician notification.
