A recertification survey was completed on 10/9/25. Deficiencies were cited. Based on observation and interview the facility (residence) failed to provide a safe environment affecting seven current members (residents). Findings include:1. ReferenceAll assisted living residences shall be in compliance with the Colorado Clean Indoor Air Act, Sections 25-14201 through 25-14-209, C.R.S.2. ObservationOn 10/8/25 during the onsite tour, at approximately 8:40 a.m., a black planter with cigarette butts in it was found near the sun room door of the residence. Next to the planter were more cigarette butts mixed with dry pine needles next to the house, approximately three feet from the door. On 10/8/25 during the onsite tour of the residence, at approximately 8:40 a.m., a tin coffee can labeled "put your butts here" was discovered outside the sun room door. 3. InterviewOn 10/8/25 at approximately 10:15 a.m., the resident care manager stated that the designated smoking area was the area outside the sun room door where the coffee can was. On 10/9/25 at approximately 11:00 a.m., the administrator stated that they used to have a cigarette dispenser; however, residents who smoked should be away from the house. THIS PORTION OF THE REPORT IS FOR INFORMATIONAL PURPOSES ONLY.No response is necessary.The residence was advised it must review and maintain the following processes in accordance with existing program regulations found at 10 CCR 2505-10 8.7000 8.7001.B.2.A.iii.1 The setting ensures an individual' s rights of privacy, dignity, and respect, and freedom from coercion and Restraint. 1) The right of privacy includes the right to be free of cameras, audio monitors, and devices that chime or otherwise alert others, including silently, when a person stands up or passes through a doorway.

Findings include:On 10/8/2025 at approximately 8:30 a.m., the 40 hour administrator training certificate was requested. The residence was unable to provide the 40 hour training certificate. On 10/8/2025 at approximately 9:00 a.m., the administrator stated that she only had the 30 hour training and was working on the additional 10 hours of training. She stated she was unaware that there was a deadline for the extra 10 hour training and she did not have the full 40 hour training. Based on record review and interview the residence failed to assign at least on staff member the responsibility for infection prevention and control program and training, affecting seven current residents. Findings include:On 10/8/2025 at approximately 8:40 a.m., the Infection Control Officer training certificate was requested. The residence was unable to provide the documentation. On 10/8/2025 at approximately 2:00 p.m., the administrator stated that she was supposed to be the one trained in infection control and in charge of the infection prevention and control program. She stated that she did not have her certificate and that she would get trained. Based on record review and interview the residence failed to document investigations and resolutions of any irregularities found on the medication administration records during the medication audit affecting seven current residents. Findings include:A document titled medication administrator record (MAR) Audit and dated August 2025 did not provide record of investigations and resolutions of irregularities marked on the record. On 10/8/2025 at approximately 4:30 p.m., the resident care manager stated that she did not know that she needed to include the investigation and resolution of irregularities and that she would add that to their MAR Audit sheet.On 10/8/2025 at a..
