The facility failed to ensure the accuracy of medication administration records (MAR) for 3 out of 3 sampled residents. Specifically, the records contained duplicate entries for medications administered.

The facility failed to ensure that furniture was in good repair. Specifically, a vinyl armchair in a resident's room had two splits in the seat cushion that extended through the batting.

The facility failed to maintain an adequate supply of paper towels or hand towels for resident use. Observations showed that all three resident bathrooms had empty paper towel dispensers, leaving residents unable to dry their hands after washing.

The facility failed to maintain clean and well-repaired walls, floors, and bathrooms. Specifically, the guest bathroom was found with a broken doorknob, blocked by furniture, lacking soap and towels, and containing debris and dead insects. Additionally, vinyl wall coverings in resident rooms were peeling, dusty, and sticky.
