The facility failed to ensure at least one staff member on duty at all times completed an accredited CPR course within the last 24 months. Specifically, three sampled staff members were found to have non-compliant training, including a staff member who completed an online-only course without in-person components and another with expired or non-accredited certification.

The facility failed to ensure that at least one staff person on the premises at all times had completed a CPR and choking management course that included hands-on training. Specifically, three sampled staff members (Staff A, Staff D, and the Administrator) provided certifications from the National CPR Foundation that were based on online modules without a return demonstration of skills. Additionally, the Administrator lacked physical documentation for an in-person training session.
