The facility failed to ensure that all required documentation was maintained and accessible within the residents' records. Specifically, for three sampled residents, documentation was missing from their files, including the absence of an FL-2 form for Resident #1.

The facility failed to ensure that required educational qualifications and documentation were on file prior to employing three staff members designated as Supervisors-in-Charge. Specifically, personnel records for Staff A and Staff B lacked evidence of high school graduation or GED certification, job descriptions, SIC applications, and required reference letters.
