The following deficiency was found during the on-site compliance inspection conducted on December 4, 2025:

Based on observation and interview, the manager failed to ensure the premises and equipment used at the assisted living facility were free from a condition or situation which may cause a resident or other individual to suffer physical injury. The deficient practice posed a threat to the physical health and safety of residents. Findings include: 1. During an environmental tour of the facility, the Compliance Officer observed a ramp in the backyard which led to another area of the fenced in backyard; however, the ramp was not in working condition as it had several big holes throughout. 2. In an exit interview, the findings were reviewed with E1. E1 acknowledged the holes in the ramp could be a tripping hazard for residents and advised they would be repaired.
