No record of a meal census (kind and amount of food used) was maintained for the preceding 3 months.
Hazardous and toxic chemicals were found in an unlocked lower cabinet and on a kitchenette counter in the memory care unit, accessible to residents.
Nine resident room refrigerators/freezers did not have a reliable thermometer.
One resident's TB screening was completed after admission rather than within 12 months prior to admission.
