Key Review Excerpts

My step-father was transfered from the hospital late Friday for rehab for a stroke. He sat in his urine for hours until my mom visited on Saturday. He was extremely thirsty and was not given his pain meds! He was NOT being cared for AT ALL!

Family member of rehab patient · 2024☆☆☆☆

The nurse refused to give me my meds when I was supposed to have them. This caused such a neurological reaction in my health that we had to call 911 and I was taken by ambulance to the ER.

Rehab patient · 2023☆☆☆☆

The activities crew went out of their way to give her things to do while we were gone. She received excellent care from the therapy staff. And nursing was always willing to give her the care and medical support she needed. The only fault of this facility is the communication between staff.

19 complaint-triggered

This facility shows concerning patterns with frequent family complaints triggering 20 of 61 deficiencies across multiple areas. Recurring issues include quality of care and treatment, infection control problems appearing in every survey since 2022, medication management errors, and safety violations. Recent complaints in 2025 involved abuse reporting failures and resident rights violations. While all deficiencies show correction dates, the persistent nature of core problems across nursing care, safety protocols, and medication handling suggests ongoing operational challenges families should carefully evaluate.
