What this means for your family

This facility offers excellent engagement through their activities program, which can significantly improve a resident's quality of life. However, families must be vigilant regarding personal belongings and daily hygiene; we recommend frequent, unannounced visits to ensure your loved one is receiving consistent care and that their assistive devices are being managed properly.

Key Review Excerpts

She really likes the lady name Melinda that is over actives down there she can color, play bingo, and cooking with Melinda is really fun, they done pizza the other day so that helps make her day go by fast.

Long-term resident's family · 2025★★★★

Dirty! Rooms in desperate need of vents cleaned, ceiling tiles stained and old from olw water leaks, rooms haven't had new paint in years. Theft has gotten bad. Patient gifts, phone, tv remotes and belongings stolen.

Family member · 2025☆☆☆☆

For the past two weeks I’ve been to visit they didn’t have her hearing aides turned on or her glasses on. And more than a few times they didn’t even put her teeth in.

Hartwell Health and Rehabilitation shows a mixed record with all 12 deficiencies corrected except one unresolved issue from 2022 regarding resident assessments. The most recent 2024 survey found multiple problems in medication management, safety hazards, and facility environment, but the provider has correction dates for all these issues. Fire safety violations appeared in earlier surveys but seem resolved, while the main concern is whether operational improvements from the latest survey have been sustained.
