Failure to maintain an organized program for resident protection: Narcotic count logbook was left blank on multiple dates (11/22/25, 11/23/25, 12/1/25, and 12/14/25), making it impossible to verify if counts were completed. Additionally, five residents had unsafe bedside assistive devices (potential entrapment risks) without physician orders, and service plans lacked necessary monitoring/maintenance instructions. The facility also lacked a policy for these devices.
