FACILITY EVALUATION REPORT

This report details a complaint investigation conducted following an unannounced visit. The investigation found the original allegation regarding staff notification of an incident to be unfounded. No deficiencies were cited during this visit per California Code of Regulations, Title 22.

The inspection was a Case Management visit concerning a deceased licensee and civil penalties. Multiple Type A deficiencies were cited, including failure to report the licensee's death promptly and issues related to the administrator's qualifications and financial standing. Immediate corrective action is required to mitigate risks to the residents.

This report details a follow-up inspection following deficiencies cited on September 3, 2025. The initial visit cited three Type A deficiencies related to Emergency Plans, Medical Assessment, and Personnel Requirements. Although the facility submitted documentation by the extended due date of 9/12/2025, the narrative indicates that no deficiencies were cited during the current unannounced POC visit on 9/19/2025.

This was an unannounced POC visit conducted by the LPA to follow up on deficiencies cited during the facility's annual continuation inspection on September 3, 2025. The facility was found to have outstanding deficiencies related to emergency plans, medical assessments, criminal record clearance, and personnel requirements. While one deficiency was cleared, the remaining Type A citations require follow-up documentation.

The inspection revealed multiple deficiencies across several critical areas, including emergency preparedness, resident recordkeeping, staff training, and staff association documentation. Several citations were classified as Type A, indicating immediate risks to the health and safety of the residents. The facility must address these compliance failures promptly to ensure safe operation.
