This facility has areas of concern that warrant careful consideration. Registered Nurse hours are 25% of the EveryPlace reference benchmark; ask how RN coverage is allocated across shifts. We recommend asking the administrator directly: "How are you addressing recent staffing shortfalls?" These are not reasons to panic, but they are reasons to ask tough questions and visit in person.
Gonzales Healthcare Center presents a stark contrast in experiences, with many visitors praising the friendly, welcoming staff and clean environment, while others report severe concerns regarding neglect and poor clinical oversight. Families frequently highlight issues with call light response times, inadequate staffing levels, and failures in wound care or medication management. Prospective families should weigh the positive reports of short-term rehab success against the serious allegations of neglect in long-term care.

Key Review Excerpts
“The nursing staff was attentive and always made sure my mom was comfortable and cared for. Her therapy team was wonderful as well she made real progress.”
“I personally , Charlene Julien want to thank some of they employee by name for help me with Mrs. Janice. I take my hat off to them: Ms. Donna Chenervert, Kalyn Green, Lakesha Barker, Charmaine Jackson, Quavonna Burton, and Royce Stewart. They were my family.”
“My mom was in Gonzalez Healthcare and when I tell you NEGLECT is an understatement. She was found unresponsive and was sent to hospital in early hours ( like 3am), I was not notified until 10pm and was told my mom had been declining for a few days so they sent her to hospital and that the nurse forgot to call me.”
Both RN and total nursing hours are below the EveryPlace reference benchmarks. Ask the facility how it staffs each shift for current resident needs.
Reference benchmarks (0.75 RN and 4.1 total nursing hours per resident/day) are comparison targets, not current federal minimum requirements.

The most persistent issues involve abuse protection, care planning, and medication management, with the same problems recurring across multiple surveys despite claimed corrections. Given the pattern of repeated complaints and the facility's apparent inability to maintain lasting improvements in critical safety areas, families should exercise extreme caution and strongly consider alternative options.

EveryPlace is a research directory. Facility information is compiled from public sources — Medicare.gov, state licensing portals, Google Places, and publicly available street-level imagery. Some summaries, interpretations, suggested questions, and pricing research are AI-generated or AI-assisted and may contain errors or omit important context. Listings do not constitute endorsement, recommendation, or advertisement, and we do not accept payment for placement.
