Key Review Excerpts
“The staff is very friendly and really care about the patients that are here no matter if its a short term or long term stay. They keep you informed about things going on recreationally and personally. The administrator is awesome! She visits with everyone while she's in the building. All the nurses and their aids are attentive as well. The kitchen makes delicious food which is a dramatic opposite of other places. Very homey vibe!!”
“My brother was accepted to their facility for long-tern care and was sent to the hospital with the expectations that they were going to accept him back once he was stable, now they are refusing to accept him back and they have failed to call the family to let us know they are not taking him back”
“I absolutely love this place and love Jesse in the evenings. My mother loves it there but sometimes in the evening or when I call the nurses to answer my moms phone for her because she can’t. The nurses are absolutely rude and always act like your bothering them.”

Inspection History
Medicare Inspection History
3-year lookback · Medicare 2026
Families filed complaint reports that triggered deficiencies in 2024 related to protecting residents from abuse and neglect, though these issues were reportedly corrected. The facility shows recurring problems with building safety systems (fire safety, heating/ventilation, electrical), resident care planning assessments, and food safety that persist across multiple surveys from 2022-2025. While most deficiencies have correction dates, the pattern of repeated violations in safety systems and ongoing heating/ventilation issues suggests potential maintenance challenges.
