Number of residents present at the facility at the beginning of the inspection: 16 The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law. Resident room #6 had two beds and held two residents. The LI could not locate any chairs in the room during the time of the inspection.

Based on observations made during the tour of the building, the facility failed to maintain all furnishings, fixtures, and equipment including sinks, toilets, bathtubs, and showers clean and in good repair and condition. EVIDECE: 1. During the inspection the bathroom across from resident room #2 had the middle shower knob off. This was the knob that you would turn to turn the shower head on and off. The knob was found by the LI to be sitting on the tub?s edge.

Based on observations made during the tour of the building, the facility failed to include a sturdy chair for each resident in two of the resident bedrooms. EVIDENCE: 1. According to staff #2, there are two residents that reside in room #6 and room #2. Both rooms (#6 and #2) only had one chair available for the residents.

Based on the tour of the facility, the facility failed to keep the interior and exterior of all buildings maintained in good repair. EVIDENCE: 1. A large piece of lattice on the back porch area of the building was found to be broken, missing, and in need of repair.

Based on observations made during the tour of the facility, the facility failed to ensure all furnishings, fixtures, and equipment, including furniture shall be kept clean and in good repair and condition. EVIDENCE: 1. In the common area beside the TV room there is a beige colored set, there was a six-eight-inch rip in the seat area of this chair. 2. In the hall area, outside of the common TV room and near the book case, a beige printed chair was found to have almost the entire area of the seat ripped.

In resident room #1, the second bed from the door was found to not have any sheets on the bed. LI asked resident why the bed did not have any sheets and he replied, ?I don?t know?.

A non-mandated monitoring inspection was initiated on 09/29/2021 and concluded on 09/29/2021. The person in charge was contacted by telephone to conduct the investigation. The licensing inspector conducted an on-site observation at the facility on 09/9/2021. The evidence gathered during the non-mandated focused monitoring inspection did not support the non-compliance with standards are laws.

A monitoring inspection was initiated on 8/16/21 and concluded on 8/19/21. The administrator was contacted by telephone to initiate the inspection. The administrator reported that the current census was 19. The inspector emailed the administrator a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed two resident records, two staff records, the activities menu, the menu, staff schedules, etc. submitted by the facility to ensure documentation was complete. The inspector conducted the on-site portion of the inspection on 8/19/21. An exit interview was conducted with the Administrator on the date of the inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliances with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Based on observations made during the tour of the building, the facility failed to have the menu for the current week dated. EVIDENCE: 1. The menu posted in the facility had the dates for 2020, not the current year of 2021.

Based on observations made during the medication cart audit, the facility failed to administer medications consistent with the standards of practice outlined in the current registered medication aide curriculum approved by the Virginia Board of Nursing. EVIDENCE: 1. Resident #1 is prescribed Levemir, Dorzolamide-Timolol Eye Drops, and Erythromycin 0.5% Eye Ointment among other medications. These three medications were opened and had been used but were not labeled with an open date. 2. The top drawer of the facility medication cart contained more than 20 different medications that were not labeled with an open date such as eye drops, ear drops, nasal sprays.
