The facility underwent an unannounced one-year required visit. The LPA conducted thorough inspections of the kitchen, bedrooms, common areas, and reviewed resident and staff files. No deficiencies were observed during the visit, and the report confirms compliance with Title 22 Division 6 of the CA Code of Regulations.

The inspection was conducted as a Case Management visit following a complaint. Multiple deficiencies were cited, all classified as Type B. Key issues include failure to update resident care plans following changes in condition and failure to submit required incident reports to the CCLD in a timely manner.
