A relicensure survey wth complaints #CO33747 and #CO36195 was completed on 10/9/24. Deficiencies were cited. Based on observation and interview, the residence failed to provide a physically safe and sanitary environment, affecting 16 current residents.Findings include:On 10/9/24 from approximately 8:00 a.m. until approximately 9:45 a.m., both common bathrooms were in unsanitary condition, with urine and feces on the toilet seats..On 10/9/24, at approximately 3:45 p.m., the administrator director stated that the expectation was that staff ensured all bathrooms were cleaned. She also noted that the condition of both bathrooms was unsanitary. Based on observation and interview, the residence failed to provide toilet paper and paper towels in each common bathroom, affecting 16 current residents.Finding include:On 10/9/24 at approximately 8:00 a.m., an environmental tour of the residence revealed that both common bathrooms did not have toilet paper or paper towels available. On 10/9/24, at 4:00 p.m., the administrator director stated that staff had informed her that they were out of toilet paper and paper towels the morning of the onsite visit. She acknowledged that the residence should have provided these supplies as required.
