The unannounced Case Management - Incident inspection was conducted on November 20, 2025, following a self-reported Death Report for Resident #1. The Licensing Program Analyst observed the facility to be clear of any hazards and noted no immediate health or safety concerns. No deficiencies were cited per Title 22 of the California Code of Regulation on this visit.
