FACILITY EVALUATION REPORT

The inspection revealed multiple deficiencies, including three Type A citations related to immediate health and safety risks concerning the grounds, unsecured sharps/knives, and general facility maintenance. Additionally, one Type B deficiency was noted regarding the failure to conduct required quarterly emergency drills. Immediate corrective action is required for all cited deficiencies.

The facility underwent a required annual inspection. The Licensing Program Analyst observed the facility and found that all areas, including resident rooms, bathrooms, and the backyard, were clean and operational. No deficiencies were cited during the visit.

The inspection was a Case Management visit concerning a death report for a client who passed away on 9/26/2020. The primary finding relates to the facility's lack of documentation or follow-up regarding the death, as the administrator reported no communication from police or the coroner. This represents a non-compliance issue regarding required reporting procedures.
