COMPLAINT INVESTIGATION REPORT

The facility underwent an unannounced Case Management visit on October 3, 2025. The visit confirmed that the facility received the Default Decision and Order for Staff 1 (S1). Staff interviews and record reviews confirmed that S1 is no longer associated with the facility, and no deficiencies were cited during the inspection.

The inspection identified multiple deficiencies across several areas, including immediate safety hazards from unsecured chemicals and medications (Type A). Additionally, there were several Type B deficiencies noted concerning facility maintenance, proper storage of food, and medication handling. Overall, the facility requires significant corrective action to ensure resident safety and compliance with regulations.

The inspection was conducted on 09/19/2025 as a 1-Year Annual Required inspection. Due to time constraints, the Licensing Program Analysts were unable to complete the full annual inspection and will return at a later date. No specific deficiencies were cited on this report.

The analyst observed that hot water temperatures were within acceptable ranges, and general safety measures such as smoke/CO detectors, first-aid kits, and fire extinguishers were in place and functional.
