What this means for your family

While some families report positive rehab outcomes, the recent surge in reports of severe neglect and hygiene issues is a major red flag. If you are considering this facility, we strongly recommend conducting an unannounced visit and specifically observing how staff respond to residents who require assistance with mobility and toileting.

Franklin Oaks Nursing and Rehabilitation Center receives highly polarized feedback, with some families praising the compassionate, professional staff and successful rehab outcomes, while others report severe neglect and staffing shortages. Recent reviews from 2025 and 2026 highlight critical concerns regarding hygiene, bedsores, and a lack of basic assistance for non-ambulatory residents. Prospective families should be aware of the significant disparity between positive experiences and reports of systemic understaffing.

Questions for Your Tour

  • 1Given the recent feedback regarding staffing levels, what specific protocols do you have in place to ensure residents receive timely assistance with daily needs like toileting and hydration?
  • 2I noticed the facility has had some recent health inspection findings; could you walk me through the steps you are taking to improve overall cleanliness and hygiene standards?
  • 3Since physical and occupational therapy are noted as strengths here, how do you coordinate those sessions with the nursing staff to ensure residents are repositioned regularly and kept comfortable?
  • 4What is your current process for medication management, and how do you communicate any changes in a resident's health status to their family members?
  • 5I see that you occasionally engage with feedback online; how does that administrative responsiveness translate into daily improvements for the residents living here?
  • 6Could you describe the daily social activities available, and how do you encourage residents who might be less mobile to participate and stay engaged?

Key Review Excerpts

The staff is negligent and does not care for their patients. They leave patients unattended for hours. For instance, one lady, who can’t walk on her own, was left in soaked undergarments for multiple hours.

Family member · 2025☆☆☆☆

My husband was there in July for three weeks of rehab after knee replacement surgery. He was gotten up once a day for therapy and then in bed the remainder of the time. When asked why he was not gotten up periodically/escorted to the bathroom to use the toilet the reply was, “we are understaffed and can’t do that now.”

Spouse of rehab patient · 2022☆☆☆☆

My father in law had a stage 4 bed sore. He was not turned to prevent this from happening. They just complained that he was not doing his PT like he should.
