Residents were not reviewed by the Registered Nurse delegator every 90 days.
The facility failed to ensure residents were administered medications as ordered by their physician for 1 of 3 residents reviewed, specifically failing to hold medication when blood pressure parameters exceeded 130.
A resident refused medication daily for two months and the physician had not been notified.
The facility did not provide proper hand washing supplies in resident care areas when hands were visibly soiled.
