Specifically, the facility did not consistently screen staff and visitors for fever and respiratory symptoms, failed to ensure staff wore facemasks at all times, and did not consistently use appropriate PPE when in contact with residents.

Specific issues included blue and black scratch marks on walls in resident rooms, a dented door with missing paint in the living room, and a dusty air return vent. Additionally, a phone jack box had become detached from the wall, and cobwebs were observed on a wall near a closet.
