Specifically, no medication reviews had been conducted since April 11, 2023, because the facility failed to contact the contracted pharmacy to schedule the required visits.

The facility failed to ensure the accuracy of Medication Administration Records (MAR) for one resident regarding the documentation of long-acting and short-acting insulin. Specifically, there were multiple periods in September 2019 where doses of Lantus and Novolog insulin were not documented as administered, despite the resident receiving the medication.
