What this means for your family

Fair Haven's rehab and independent living programs are highly regarded, particularly for their therapy and dining services. However, families considering long-term or memory care should be extremely cautious; please conduct unannounced visits and specifically ask about current CNA-to-patient ratios and the facility's plan for addressing hygiene and room maintenance.

Fair Haven presents a stark divide between its well-regarded independent living and rehab services and its struggling long-term and memory care units. While many families praise specific staff members and the quality of food, there is a recurring pattern of severe neglect, poor hygiene, and communication failures in the nursing care sections. Prospective families should be aware that while some find the facility excellent, others report significant concerns regarding staffing levels and patient dignity.

Key Review Excerpts

My Aunt was there on rehab second floor through the holidays and I must say that her care taker Wanda was very attentive and caring, and not to mention the awesome cook Jennifer is a exceptional cook and very polite as well as professional.

Long-term resident's family · 2025★★★★★

My father-in-law recently stayed in Fair Haven Rehab floor 2. Many people were very attentive to him and always treated him with kindness. This was certainly a hard time for our family going through this, but Erika the social worker and the nurses like Simone and Jessica made it so much easier on us.

Rehab patient family · 2025★★★★★

The nursing home has a nice ambiance. Too bad the staff doesn’t line up with with it looks it. May have been 3 nice staff members. CNAs are extremely unprofessional, and rude.

Fair Haven has corrected all 26 deficiencies found across three routine surveys, with no complaint-triggered inspections. The facility had notable issues with building safety systems (fire exits, sprinkler systems, electrical), care planning and assessments, and infection control that appeared across multiple surveys. While all violations have correction dates on file, the pattern of recurring building safety and care coordination problems suggests families should ask about recent improvements during their visit.
