What this means for your family
This facility is an excellent choice if you are looking for a warm, home-like environment with a staff that treats residents with genuine dignity. The high level of care for dementia and post-injury recovery is a standout feature. Since the facility operates in a more intimate group-home setting, it is a great fit for those seeking personalized attention rather than a large institutional campus.

State Inspection History
State Inspections

Apr 25, 2024Routine
The following deficiency was found during the on-site compliance inspection conducted on April 25, 2024:
Based on documentation review, record review and interview, the manager failed to ensure a caregiver provided current documentation of first aid training and cardiopulmonary resuscitation (CPR) training certification specific to adults for two of three caregivers reviewed. The deficient practice posed a risk if an employee was unable to meet a resident's needs during an emergency. Findings include: 1. Review of the facility's policies and procedures revealed a policy titled "CPR, First AID". This policy stated " Extended Family Homes requires that a caregiver obtains and provides documentation of cardiopulmonary resuscitation training specific to adults, which includes a demonstration of the caregiver's ability to perform cardiopulmonary resuscitation...." 2. Review of E3's personnel record revealed a first aid card and CPR card which expired in January of 2024. 3. Review of E4's CPR card revealed that E4's CPR card was issued online through the National CPR Foundation, which is an online-only training program that did not include a demonstration of the caregiver's ability to perform CPR. 3. Review of the April 2024 personnel schedule revealed E3 worked the day shift on April 25, 2024 and E4 worked the day shift on April 24, 2024. 4. In an interview, E1 confirmed that E3 did not have a current CPR card and that E4 acquired their certification through an online program.
