What this means for your family

This facility offers an excellent physical therapy program that can significantly aid in recovery, but families must be vigilant regarding daily care. We strongly recommend that you visit frequently at different times of the day, especially on weekends, to monitor call bell response times and ensure your loved one's basic hygiene and medication needs are being met consistently.

Excel at Woodbury is a polarizing facility that receives high praise for its physical therapy department and clean, modern environment, but faces serious allegations regarding nursing responsiveness and basic patient care. While many families report successful rehab outcomes and attentive staff, a recurring subset of reviews describes significant neglect, including unanswered call bells, hygiene issues, and medication errors. Families considering this facility should weigh the strong clinical rehab results against the reported inconsistencies in daily nursing care.

Key Review Excerpts

The therapists have one thing in their mind and THAT IS TO GET YOU BETTER AND SEND YOU HOME AS QUICKLY AS POSSIBLE. Approximately 9 days later, I was using the walker and going up and down the hallways.

Rehab patient · 2024★★★★★

Do I mention the numerous times she pressed the call bell, which went unanswered, that’s intended to alert aides when she’s in need of assistance? In her case it was countless times to get to the bathroom bc of her inability to do so independently.

Memory care family member · 2025☆☆☆☆

The staff went out of their way to make sure he was comfortable and had everything he needed. The Physical Therapy staff worked closely with my husband, twice daily!

1 complaint-triggered

This facility has concerning patterns with 23 deficiencies across multiple areas, including a complaint-triggered investigation where families reported suspected abuse or neglect. The most recurring issues involve care planning (appearing in all three surveys), fire safety systems, and medication management. While all deficiencies show correction dates, the persistence of care planning problems across 2022, 2024, and 2025 suggests ongoing challenges in developing proper resident care plans.
