The facility failed to ensure that prescription medications were not transferred from one container to another except when prepared for administration to a resident. A Medication Aide removed pregabalin capsules from a pharmacy-dispensed bubble pack and placed them into multidose packages to make administration easier. This practice resulted in medications being stored in unauthorized containers that were not prepared by the dispensing pharmacy.

Specifically, for Resident #1, medications were found in multidose packages that had been opened and contained split tablets and capsules not matching the original dispensed quantity.
