Multiple issues observed: unsanitary bathrooms, debris/stains in common areas, structural damage including water-damaged ceiling, broken carpet, holes in exterior stairwell and floor, trip hazards from damaged stairs/tiles, and lack of cleaning schedules for kitchen equipment.
Facility failed to ensure 2 of 4 staff members had valid, renewed Washington State background checks completed every two years.
Staff A could not remember why their TB test was late. Staff B did not have a TB test at the time of hire. Director of Nursing stated a better system is needed for testing within 3 days of hire.
