Client 5's bedroom window was blocked by a headboard, and a sharp barbecue fork was left unsecured in a home where knives/forks were required to be locked.
Provider failed to immediately report alleged abuse of Client 3 (physical holds and hitting a housemate) to the Complaint Resolution Unit.
Provider failed to implement health professional orders regarding diet (soft and bite-sized) for Client 5, resulting in the client consuming improper food.
Provider failed to obtain complete FBI RAP sheets for Staff A as part of the character, suitability, and competence review.
