The facility failed to ensure one of three sampled residents had completed the required two-step tuberculosis (TB) testing. Specifically, there was no documentation of a TB skin test in the resident's record upon admission.

The facility failed to administer medications as ordered to one of three sampled residents. Specifically, the resident was not receiving their prescribed albuterol HFA inhaler as required by the physician's orders.

The facility failed to ensure one of three sampled residents had completed the required two-step tuberculosis (TB) testing upon admission. Record reviews showed no documentation of a TB skin test for Resident #1, and the Administrator could not provide evidence that the test had been performed by the primary care provider.

The facility failed to ensure that the preparation and administration of medications and treatments by staff were in accordance with orders by a licensed prescribing practitioner and facility policies.

The facility failed to ensure therapeutic diets were served as ordered for a resident who required a nutritional supplement three times daily. Observations and medication administration records showed the resident did not receive the prescribed nutritional shakes during breakfast, lunch, or snack times.

The facility failed to ensure a Licensed Health Professional Support (LHPS) evaluation was completed for a resident requiring assistance with transferring and ambulation. The last documented evaluation for the resident was dated 03/03/17, failing to meet the requirement for evaluations within the first 30 days of admission or at least quarterly thereafter.
