Based on observations made during a tour of the building, the facility failed to ensure each bedroom shall contain all required items, including a sturdy chair for each resident. EVIDENCE: 1. Resident rooms #209, #104 and #109 were assigned two residents; there was only one sturdy chair observed in each of those rooms. 2. Resident room #110 was assigned one resident; there were no chairs observed in the room.
Based on observations made during a tour of the building, the facility failed to ensure hot water at taps available to residents shall be maintained within a range of 105?F to 120?F. EVIDENCE: 1. In resident room #205, the hot water at the sink reached a temperature of only 94 degrees Fahrenheit.
Based on observations made during a tour of the building, the facility failed to ensure the interior and exterior of all buildings shall be maintained in good repair and kept clean and free of rubbish. EVIDENCE: 1. A small stain was observed on the carpet in resident room #300 in front of the closets, covering an area of approximately 3 by 5 inches. 2. A small dark stain was observed on the carpet in resident room #304 on the left by the baseboard next to the entrance, covering an area of approximately 3 by 4 inches.

In the enclosed outdoor area by the sitting room at end of the special care unit, a portion of the black chain link fence that joins the black aluminum fence had separated from the post, leaving a gap of approximately 10-12 inches of open space.

Based on a review of resident records, the facility failed to ensure that physician's or other prescriber's oral orders shall be reviewed and signed by a physician or other prescriber within 14 days. EVIDENCE: 1. Per facility nurses? notes dated 04/11/2024, a palliative care provider was in the facility the same date to see resident #1 for monthly rounds. When asked about a wound on the resident?s back, the provider stated she sent ?new orders for Medi honey twice weekly? to the home health provider. 2. Per documentation provided by the facility, the prescriber?s verbal order was given on 04/10/2024 and stated the following: ?SN to cleanse wound with NS, pat dry, apply Thera honey, and secure with Opti foam dressing twice weekly and as needed for loosening/soiling.? 3. The documented order was signed by the prescriber on 05/14/2024, more than 30 days after the verbal order was given.
