The facility failed to implement and maintain CDC and NC DHHS guidance regarding the use of personal protective equipment (PPE) during the COVID-19 pandemic. Observations showed staff members answering doors, assisting residents, and working in common areas without face masks or with masks improperly worn below the nose. Additionally, staff were observed removing masks after meal services and while outdoors, and the facility lacked sufficient PPE supplies.

The facility failed to implement a physician's order for weekly blood pressure checks for one resident. Review of medication administration records showed no blood pressure readings were recorded for October 2018, and only one reading was documented in November 2018.
