The analyst observed that the Resident Centrally Stored Medication and Destruction Record (CSMDR) was maintained in each resident's file for all 8 residents. All deficiencies cited during the previous annual visit on 10/2/2025 were reported as cleared.

The facility was inspected on 09/21/2021 and was observed to be clean, in good repair, and meeting basic safety standards. The report explicitly states that no deficiencies were found per California Code of Regulations, Title 22 Division 6, Chapter 8. However, the administrator was asked to submit updated forms (LIC 610, Copy of liability insurance) by a future date.
