The facility underwent an unannounced visit for a case management Plan of Correction (POC) review. The analyst observed that the Resident Centrally Stored Medication and Destruction Record (CSMDR) was maintained in each resident's file for all 8 residents. All deficiencies cited during the previous annual visit on 10/2/2025 were reported as cleared.
