The facility failed to notify the Department of Social Services regarding an incident involving Resident #2, who suffered a fall on 11/06/23. Although the resident required evaluation at the emergency department due to scalp swelling, the facility did not complete an accident/incident report or notify DSS. The Administrator stated she did not report the fall because she believed notification was unnecessary if no injury was present.

The facility failed to ensure that all residents received required tuberculosis testing upon admission and subsequent testing. Specifically, two residents failed to receive their required 2nd-step TB skin test, and one resident did not receive a TB skin test at the time of admission.

The facility failed to ensure that 2 out of 3 sampled residents received their required 2nd-step TB skin test and failed to ensure 1 resident received a TB skin test upon admission. Specifically, records for Resident #1 showed no evidence of a 2nd-step test, and Resident #3 lacked a TB skin test following their recent admission.
