What this means for your family

Crystal Oaks has a strong reputation for its rehab therapy team and compassionate nursing care, making it a potential candidate for short-term recovery. However, families must be vigilant regarding recent reports of staffing shortages and maintenance issues; we strongly recommend touring the facility and asking specifically about current response times for call buttons and the status of the heating system.

Crystal Oaks presents a polarized experience, with many families praising the compassionate nursing staff and effective rehab services, while others report significant declines in quality following a recent change in ownership. Critical concerns include inconsistent responsiveness to call buttons, maintenance issues such as faulty heating, and serious lapses in medication and dietary management. Families should be aware that while some residents receive high-quality, attentive care, others have experienced neglect and administrative failures.

Key Review Excerpts

My aunt was on the memory floor and was there for almost 1 year. She had the best nurses. Shannon, Sue, and Bre were her favorites ( and mine). They gave her superior care.

Memory care family member · 2026★★★★★

This place has gone downhill in the last year since ownership changed. They don't clean the rooms. They don't pick up the laundry. And if I hadn't been there one evening to tell my mother it was time for dinner they probably would not have fed her.

Memory care family member · 2024☆☆☆☆

The worse experience I have ever had. My wife was there, and we could not get any service from the nursing staff. Push the call button and one and half hour later no one come.

A family filed a complaint about significant medication errors, and Crystal Oaks has ongoing safety and operational issues that require attention. The facility repeatedly struggles with fire safety systems, infection control, and food safety, with fire safety problems persisting across multiple surveys from 2023 to 2024. While all deficiencies show correction dates, the pattern of recurring fire safety violations suggests systemic challenges in maintaining proper safety standards.
