What this means for your family

Creekwood is highly regarded for its physical environment and administrative efficiency, making it a strong choice for those needing help with insurance and billing. However, families should be aware of reports regarding inconsistent staff responsiveness to patient needs. We recommend visiting during off-hours to observe how call lights are handled and asking specifically about their communication protocols for medical emergencies.

Key Review Excerpts

She was happy there (even tho she complained about the food. She was on mechanical ground and no one would like that). Pam was her favorite nurse. She enjoyed the group therapy, but not the physical part because she was in so much pain. Amanda in the business office was a God send to me, guiding me thru all the insurance stuff and never giving up.

Long-term resident's family · 2025★★★★★

The staff and administrators at CWN facility did an outstanding job in taking care of my 93 yr old mother for the short term she was at their facility. Their Social Services point of contact, Tiffany was on point with detailed information; she was also mom's assigned Angel; Candace, the RN, was professional and helpful; Carrie, Business Mgr., navigated a billing issue we were having w/Anthem BCBS; she got it resolved in a timely manner.

Short-term rehab family member · 2021★★★★★

They are very unprofessional when keeping stuff confidential; they walk around and tell other staff members and patients what she is saying. They are letting her lay there without a bath or take their time when she needs to go too the bathroom. I would not recommend this place for a family member.

Creekwood Nursing & Rehabilitation shows a pattern of recurring issues with infection control and food service safety appearing across multiple surveys, including a recent October 2024 inspection that found problems with medical gas storage, treatment planning, and medication management. The facility had significant care deficiencies in 2018 involving resident rights, nutrition, and dementia care, but most issues appear to be corrected by the provider after identification.
