Key Review Excerpts

The staff is super friendly and they explain stuff to you in layman's terms because not all of us are medically knowledgeable and they seem to be taking very good care of my mother while she's there for rehab

Rehab patient family member · 2023★★★★★

Unfortunately this could be a very nice facility but because of being understaffed the patients suffer. One day their was only 1 aide for 26 patients and she was so upset she started to cry when I asked her why my mom was all crooked in bed and had food and garbage all of the floor.

Long-term resident's family · 2022★★★★

My grandma spent the last months of her life at a Countryside and the every time I asked her how things were, she said she really loved it. The staff were wonderful with her while we were there, and her room was always clean.

This facility shows concerning patterns with 54 deficiencies across six surveys and 10 complaint-triggered violations from families. Primary issues involve safety hazards and accident prevention (appearing across multiple surveys), fire safety systems, and basic care quality including wound care and incontinence management. While most deficiencies show correction dates, the recurring safety problems and multiple family complaints suggest ongoing challenges with maintaining consistent care standards.
