Key Review Excerpts

Most seriously, my mother has received incorrect medications on THREE separate occassions. Luckily she was coherent and realized she had been given the wrong cup of pills the first two times. Today she was given, and took, someone elses meds.

Long-term resident's family · 2022★★☆☆☆

I was DNRed (Do Not Return) for calling for help on a Saturday morning, when I had 28 Residents, all by myself. I called the Administrator at 7:00am… apparently, that was a punishable offense. There was Black Mold in the Ice Machine, too.

Former employee · 2025☆☆☆☆

Staff everywhere were professional, patient, and kind as her dementia progressed, even when she was difficult to deal with. Many staff members have worked there for several years, which speaks to the friendly, caring environment at CGP.

17 complaint-triggered

Cottage Grove Place has a concerning pattern of complaint-driven deficiencies, with families filing 16 reports leading to federal violations across multiple areas. The facility has repeatedly struggled with accident prevention and safety hazards, nursing staffing levels, and medication management, with safety issues appearing in three separate surveys despite corrections. While all deficiencies show correction dates, the recurring nature of core care problems suggests persistent operational challenges that warrant careful consideration during any visit.
