What this means for your family

Concordia offers excellent therapy services and a clean environment, making it a strong candidate for short-term rehab. However, due to multiple reports of delayed response times and inconsistent wound care, we strongly advise families to visit frequently and maintain clear, documented communication with the nursing supervisor regarding your loved one's specific care needs.

Concordia Lutheran Health and Human Care receives highly polarized feedback, with many families praising the compassionate, skilled nursing and therapy teams, while others report significant issues with chronic understaffing. While many residents and families describe a clean, professional environment, recurring complaints highlight delayed responses to call bells, inconsistent wound care, and poor communication with medical providers. Families should be aware that experiences appear to vary significantly depending on the specific unit and staffing levels at the time of stay.

Key Review Excerpts

My mom has been in the personal care area of Concordia at Cabot for approximately 2 1/2 years. We are blessed to have a loving, caring and meaningful facility where she can feel comfortable and safe.

Personal care family member · 2025★★★★★

My two star rating is because the nursing staff was not in compliance with getting pain meds on time when I asked for them. There were times I waited 30 to 45 minutes for a pain pill and by the time I got it I was in agony.

Rehab patient · 2024★★☆☆☆

My dad had a stroke and traumatic brain injury in the spring... He had excellent home care from Concordia Visiting Nurses and Therapists (OT, PT, and Speech). And dad spent a couple weeks at Concordia in Cabot for therapy.

This facility has recurring deficiencies across multiple areas, with families filing complaints that triggered inspections in five cases. The most persistent problems involve resident care planning, medication management, and food safety, with food handling violations appearing in three consecutive surveys. Issues with nursing staff training, infection control, and building safety also repeat frequently. While all deficiencies show correction dates, the pattern of recurring problems in core care areas suggests ongoing challenges with maintaining consistent standards.
