What this means for your family

This facility's rehab therapy team is frequently praised for helping patients regain mobility, making it a potential option for short-term recovery. However, families should be aware of significant, recurring reports regarding facility odors and inconsistent staffing levels. We strongly recommend visiting during off-hours to observe the environment and asking management specifically about their plan to address sanitation and weekend staffing.

Key Review Excerpts

The rehab staff was great my son couldn’t even move his leg when he got there and the first day they helped to learn how to get out of bed and into a wheelchair so he could use the bathroom on his own.

Rehab patient's family · 2024★★★★★

My mom was there for rehab not even a full week and one night they had run out of food to serve for the patients. My mom has diabetes and even if she didn’t it is completely unacceptable to not have enough food for the clients within the facility.

Short-term rehab family member · 2024☆☆☆☆

It’s not perfect. Some days there are agency aides who don't get her out of bed or dress her properly. But on the days the normal staff are there she is clean and happy.

Long-term resident's family · 2024★★★★★

$41,098 in fines

This facility has faced significant quality concerns with 45 deficiencies across five surveys, including nine complaint-triggered issues where families reported problems. Most concerning are recurring deficiencies in nutrition and food services, medication management, and safety standards that persist across multiple surveys from 2022 through 2025. While the facility reports correcting issues, the pattern of repeated problems in critical care areas like wound care, accident prevention, and resident protection suggests ongoing operational challenges that families should carefully evaluate before choosing this facility.
