Based on a tour of the building, the facility failed to ensure that when there are indications that ordinary materials or objects may be harmful to a resident, these materials or objects shall be inaccessible to the resident except under staff supervision. EVIDENCE: 1. In the safe, secure environment there was a small box of objects observed in the enrichment area where residents can participate in simulated activities such as cooking and grocery shopping. The box contained replicated food items as well as fragments of two latex balloons. 2. The balloon fragments could pose a choking hazard to residents. 3. The LI gave the balloon fragments to a staff member who discarded them at the time of inspection.
Based on review of staff records, the facility failed to ensure one direct care staff attended at least 18 hours of training annually. EVIDENCE: 1. Staff #1 was hired on 09/22/1996. From September 1, 2024-September 1, 2025, staff #1 completed eight hours of annual training.
Based on review of staff records, the facility failed to ensure each direct care staff member receives certification in first aid within 60 days of employment. EVIDCENCE: 1. Staff #2 was hired on 07/07/2025 and staff #3 was hired on 06/26/2025, both were hired as direct care staff. 2. On the date of the inspection (09/18/2025) neither staff #2 nor staff #3 had received first aid training.
Based on review of resident records, the facility failed to include an answer to the statement that specifies whether the individual is or is not capable of self-administering medication on the physical exam that is completed within 30 days preceding admission. EVIDENCE: 1. Resident #7 was admitted to the facility on 10/10/2024 and had a physical completed on 10/09/2024. The physician did not answer the statement that specifies if the resident is or is not capable of self-administering medication.
Based on a review of resident records, and interview with staff, the facility failed to ensure that the resident's record shall contain the physician's or other prescriber's signed written order or a dated notation of the physician's or other prescriber's oral order. EVIDENCE: 1. The record for resident # 9 contained the following order: Clonidine 0.1mg tablet, Take one tablet by mouth three times daily as needed for SBP greater than 160. 2. There was no order in the record for resident # 9 instructing staff to obtain daily blood pressure readings and according to staff # 4, daily blood pressure readings were not being obtained for resident # 9.

Based on a tour of the building, the facility failed to ensure that bedrooms contain all required items. EVIDENCE: 1. The following resident rooms did not contain an operable bed lamp or bedside light accessible to each resident: A8, C6, C10, and C11. 2. Resident room C14 contained only one bedside light for two residents. There was no lamp by the bed near the wall.
Based on a tour of the building, the facility failed to ensure that the interior and exterior of all buildings shall be maintained in good repair and kept clean and free of rubbish. EVIDENCE: 1. The door and doorframe located on the right before the serenity room in the safe secure environment was scuffed and scratched, especially in the middle below the window and at the bottom.

Based on facility documentation and staff interviews the licensee failed to ensure compliance with the facility?s own policies and procedures. EVIDENE: 1. According to a self-report received on 08/07/2025 resident #1 was sent out to the Emergency Room (ER) with complaints of head, right wrist and left hip pain, that resulted after resident #1 was placed in a mechanical lift and the mechanical lift turned over when a wheel on the mechanical lift became caught on a fall mat. The mat caused the mechanical lift to tip and fall onto the floor while resident #1 was still in the sling of the mechanical lift. Resident #1 was returned to the facility via EMS transport with no new orders and no injuries noted. 2. During interviews with staff #1 and staff #2 both staff stated they were in a hurry as it was right before lunchtime, and they were trying to get resident #1 cleaned up and ready to go to lunch. Both staff #s 1 and 2 stated they realized they should have cleared the path/removed the mat from the floor before rolling the lift, but did not do so, which in turn caused the lift to tip while resident #1 was still in the sling of the lift. 3. The resident care policy known as RC-VA 12.080-(CL17) Use of Mechanical Lifts states, ?always follow the manufacturer?s recommendations.? 4. When referring to the manufacturer?s recommendations for the mechanical lift involved in this self-report it states under section 1 general guidelines, ?During transfer, with patient suspended in a sling attached to the lift, DO NOT roll caster base over objects such as carpet, raised carpet bindings, door frames, or any uneven surfaces or obstacles that would create an imbalance of the patient lift and could cause the patient lift to tip over.?
